LL-37

Untested in people for infections, mold illness or Lyme disease.

A natural defense peptide made by skin and white blood cells. It kills microbes in a dish and steers inflammation. Its one sizable human trial, a gel for leg ulcers, failed. A , also called Cathelicidin and CAP-18 fragment.

What the evidence shows

Fighting infections
Lab only
Lab studies only. Injections for infections, mold illness or Lyme disease have not been tested in people.
Hard-to-heal leg ulcers (topical)
Failed
In a 148-patient trial, ulcers closed in about 25% of people on either dose and 25% on placebo.

Side effects and risks

Unknown
FDA cites lab and animal findings of harm to male fertility and of tumor promotion in some tissues.
Unknown
Risk of immune reactions to injected peptide and its impurities.

Who should avoid it: Injected use. Men trying to conceive should be especially cautious.

How it works

Where it acts
It punches holes in microbe membranes and signals to immune cells.
What it changes
It kills microbes in a dish, and can calm or fuel inflammation depending on the setting.
What people notice
A gel for leg ulcers failed in a trial. High levels are linked to inflammation in psoriasis and rosacea.

How it has been given in studies

Applied to the wound bed twice a week for 13 weeks. There is no established dose for injection.

Mixed resulttrial result in rare group - 2020

The company's later subgroup analysis reported complete healing in about 29 percent of large ulcers on the lower LL-37 strength against 8 percent on placebo. It admitted the subgroup analysis was not in the original statistical plan.

Cause Of Skin Condition Rosacea Discovered

University of California, San Diego

UC San Diego research linking excess cathelicidin and its processed peptides to rosacea inflammation, showing these peptides can drive disease.

Shows cathelicidin peptides can cause inflammation, not only fight germs

Hard-to-heal venous leg ulcers

Who
Adults whose leg ulcers stay open despite standard wound care, treated with LL-37 applied to the wound in trials.
Evidence
One phase 2b trial (148 treated, 144 in the main analysis) missed its main endpoint: 13 patients in each of the three arms reached confirmed complete closure. No serious drug-related events were reported.

Legal last-resort routes

Expanded access (compassionate use)

For a serious or life-threatening condition with no comparable alternative and no trial you can join. Your physician asks the company for the drug, then files with FDA and an ethics board. The company can refuse. Costs are often not insured.

Right to Try

For a life-threatening condition after approved options are exhausted and no trial is open to you. The drug must have finished Phase 1 and be in active development. Your doctor asks the manufacturer directly. FDA does not review requests.

Trial finders

Search ClinicalTrials.gov by condition or drug name, check eligibility and call the listed site. ResearchMatch is a free NIH-funded registry that lets researchers find you. Discuss risks with your own clinician before enrolling.

Personal import for serious conditions

FDA may not object when a US citizen imports up to about three months of a drug for a serious condition with no effective US treatment, with written personal-use confirmation and a US doctor's details. It is discretion, not a right.

Patient-specific compounding

When the approved product cannot meet a specific need of an identified patient, a prescriber can order a compounded version from a licensed pharmacy. Lower price alone does not qualify. Ingredients must be pharmaceutical grade.
